Diabetes Food Pantry Program

DRIVE-THRU PANTRY ONLY– Educational material on different diabetes topics will be distributed with food pickup.

Pantry Days:

3rd Thursday of each month (Must qualify- see qualifications below)

Pantry Times:

9:00 am – 11:00 pm

Qualifications

• Diabetes medication (pills or insulin) with your name
• Prescription for diabetes medication
• Lab work with an A1c
• Hospital discharge documentation

Please bring in an ID
Diagnosis of pre-diabetes will not be enrolled
